Program Eligibility

Eligibility for the hearing aid assistance program is determined by household income and extenuating personal and financial circumstances. We use the U.S. Federal Poverty Guidelines to determine fees and services. Individuals whose household incomes fall at or below the poverty level pay nothing for services and receive battery assistance. Individuals whose household income falls between 25 percent and 100 percent above poverty are required to pay for each visit and hearing aid, according to a sliding scale payment.

Once approved as a patient of the Foundation, services include an initial hearing test and Audiological evaluation to determine loss, earmold impressions, and hearing aids. Each patient is approved for three years. At the end of the three years, patients are eligible to reapply for services.
